La narrativa de Istrati se organiza en torno a la vida de Adrian Zograffi, su alter ego imaginario que actúa como narrador, testigo o personaje. «Kyra Kyralina» y «El tío Anghel», sus dos obras cumbre, forman parte del primer ciclo. «Kyra Kyralina» (1923) abre la epopeya balcánica y recoge la vida de Stavros, su infancia, su perversión por un turco y la búsqueda febril de su hermana Kyra por los harenes de Constantinopla. La obra, que gira en torno a la idea del viaje y de la fatalidad del destino, es un canto a la amistad verdadera y a la libertad. «El tío Anghel» (1924), estructurada en tres partes independientes gracias a una cronología aleatoria, presenta el devenir trágico de dos seres excepcionales por sus pasiones excesivas: tío Anghel y el bandolero Cosma. Istrati, con la espontaneidad del cuentista oriental, aspira a que sus novelas palpiten como un corazón y combina para ello las fuentes del mejor folklore balcánico con las anécdotas y personajes de su vida aventurera. Como señala Claudio Magris en «El Danubio», es el poeta de la promiscuidad y de la ambivalencia de Oriente, de ese desorden del cual parece esperarse a un tiempo la redención y la violencia.
