La acción de la novela se desarrolla dentro de un hospital psiquiátrico, en el Estado de Oregón, donde bajo la estricta dirección y supervisión de la enfermera jefe, los pacientes de la institución, clasificados por su «grado de locura», se someten pacientemente a la pétrea disciplina de las reglas establecidas dentro de un clima de aparente orden y tranquilidad. Hasta que un día llega un nuevo interno, Randle McMurphy, el protagonista principal de la historia, quien haciéndose pasar por demente consigue ser trasladado a esta institución desde una prisión de trabajos forzados. Su carácter jovial y dicharachero, y su fuerte personalidad que contrasta con la del resto de los internos, harán que poco a poco se convierta en el «líder» del grupo, desorganizando por completo la tranquila rutina a la que la enfermera jefe, conocida como La Gran Enfermera, los tenía sometidos. Desde ese momento se creará un ambiente de tensión entre los pacientes y el personal encargado de su cuidado, que nos acompañara a lo largo de toda la historia.
